Waiver of Liability

Does anyone have a sample of a waiver of liability? It is for a contractor doing minor work, he is doing me a favor for a difficult tenant, and does not want to be sued. I am not interested in whether this is a good idea, as it's a long story, just whether anyone has one they've used they can send on.
